2	time removed	0;3	DATE-TIME			This is the date and time that the monitor was removed from the patient. Times entered without a date will be converted to the date of operation at that time.
3	applied by	0;4	POINTER	200		This is the name of the person responsible for applying the monitor to the patient. Although optional, this information may be useful in documentation for this case.
