This file contains the list of Clinics that can be used by an institution in the Wait List (Sch/PCMM) package. The clinic must be an active clinic in the Hospital Location file. Each entry in this file will contain a wait list activation date and inactivate date.

1	date activated(+)	0;2	DATE-TIME			Date the clinic is available to Wait List(Sch/PCMM).
2	activation date entered by	0;3	POINTER	200		User entering activation date for clinic to Wait List(Sch/PCMM).
3	date inactivated	0;4	DATE-TIME			Date clinic can no longer be used by the Wait List(Sch/PCMM). It cannot be earlier than the activation date.
4	inactivation date entered by	0;5	POINTER	200		Date the clinic is no longer to be used by Wait List(Sch/PCMM).
