.01	allowable modifiers	0;1	FREE TEXT		B	The ALLOWABLE MODIFIERS field contains the letter modifiers for a CPT procedure code. CPT-4 codes for Audiology and Speech Pathology may not adequately describe the scope or variety of procedures seen by audiologists and speech pathologists. Recognizing this deficiency, modifiers have been developed for certain codes to clarify procedures or to provide codes for procedures where no codes now exist. New modifiers should only be added at the direction of the Director, Audiology and Speech Pathology Service (VAHQ).
.02	description	0;2	FREE TEXT			The DESCRIPTION field contains the descriptor for the modifier of the CPT-4 procedure code. This descriptor should only be edited at the direction of the Director, Audiology and Speech Pathology Service (VAHQ).
.03	cost	0;3	NUMERIC			The COST field contains the current unit cost for each CPT-4 procedure modifier. You can use average community costs from private sector hospitals and clinics, Medicare reimbursement rates, or insurance UCRs. You can also enter real costs, if these are known.
.04	*card code	0;4	NUMERIC			The CARD CODE field has been marked for deletion. Card code capability was not implemented in QUASAR V. 1.0. Eighteen months after the release of QUASAR V. 2.0, this field can be removed.
