.02	va form 10-2633	0;2	SET OF CODES	1:YES 2:POSSIBLE		Indicates whether form VA 10-2633 is required for this incident.
.04	investigation of the incident	0;4	SET OF CODES	1:YES 2:POSSIBLE		Indicates whether the incident requires an investigation.
.06	other qa report forms	1;0	MULTIPLE	742.16		Contains the applicable QA Report Form(s) for this incident.
.07	screen status	0;6	SET OF CODES	1:INACTIVE N:NATIONAL L:LOCAL		Contains the status for the incident.
