.01	field	0;1	POINTER	78.7	B	This is the field selected from the 28 defined in the 'Label Print Fields' file. If the field is to be printed twice, it should be enclosed in double quotes. This is of particular interest if more than one 'free text' field is desired.
2	row	0;2	NUMERIC			This field defines which row the field will be printed on. It is site-specific and must be a number between 1 and 66.
3	column	0;3	NUMERIC			This field defines which column the field's title will start on. It is site-specific and must be a number between 1 and 80. If no title is to be printed, then the actual field value will start on this column.
4	title (optional)	0;4	FREE TEXT			This field contains the title to be printed for this field. It must be between 1 and 15 characters in length. A default value for this field is filled in by the system if no title is entered. If no title is desired, then the value 'NONE' should be entered in this field.
5	literal text	0;5	FREE TEXT			Text, up to 80 characters in length, may be entered here for 'free text' field types. For example, 'VA Medical Center' may be typed here to appear in the report header.
