This is a standard file exported with the DSS Extracts software. No additions, deletions, or modifications to entries in this file are permitted. As updates become necessary, a new version of this file will be released nationally.

3	inactivation date	2;1	DATE-TIME			This field contains the date when the NATIONAL CLINIC code was inactivated.
