For example, entries may be references to clinics or hospital services (e.g., General Medicine Clinic, Triage), community activities or special events (e.g., health fairs), specific individuals, and newspapers advertisements or articles. Additional information may be added to this file by the case manager through the Add/Edit to Referral Source File option. This file is also referenced through the Edit/Print Patient Case Data to document a specific referral source for a patient.

1	status(+)	0;2	SET OF CODES	1:ACTIVE 0:INACTIVE		This field stores a set of codes (1 = active and 0 = inactive) used to display or hide an entry in a selection list. If the field is active, users of this package can select the entry.
2	uppercase referral source		COMPUTED			This field converts the referral source to all capital letters for x-referencing.
