5	provider(+)	0;5	POINTER	200		Enter the name of the provider who has ordered this admission.
6	surgery	0;6	BOOLEAN	Y:YES N:NO		Answer YES if surgery will be required as part of the treatement this patient is scheduled for.
7	opt/nsc status	0;7	BOOLEAN	1:YES 0:NO		Enter yes if this patient will be admitted under the movement type OPT-NSC. Otherwise, answer no.
8	ward(+)	0;8	POINTER	42	ARSV	Enter the name of the ward this patient will be admitted to.
9	treating specialty(+)	0;9	POINTER	45.7		Enter the name of the treating specialty under which this patient will be treated.
10	ward or treating specialty(+)	0;10	SET OF CODES	W:WARD T:TREATING SPECIALTY		Respond W if this patient is being scheduled for an admission to a particular ward or T if the sheduled admission is for a particular treating specialty.
11	person scheduling admit	0;11	POINTER	200		The name of the user entering the scheduled admission will be stored here.

13	date/time cancelled	0;13	DATE-TIME			If this scheduled admission was cancelled, enter the date/time it was cancelled.
14	cancelled by	0;14	POINTER	200		If this scheduled admission was cancelled, this field will contain the name of the user that cancelled it.
15	reason cancelled	0;15	SET OF CODES	1:EXPIRED 2:OVERDUE 3:REFUSED ADMISSION 4:NO AVAILABLE BEDS 5:OTHER		Choose from the available list the reason this scheduled admission was cancelled.
16	was patient notified	0;16	SET OF CODES	0:YES 1:NO 2:NOT APPLICABLE		If the scheduled admission was cancelled, enter YES here if the patient was notified of the cancellation. Otherwise answer NO.
17	admitted	0;17	POINTER	405	AMVT	This field will contain the admission entry in the PATIENT MOVEMENT file. If this field is not filled in, the patient was not admitted for this scheduled admission.
